The Lifeskills Lab is an occupational therapy practice based in Melbourne's Western suburbs. When this policy refers to "we", "us" or "our", it means the practice and the occupational therapist responsible for your care.

We only collect information that is necessary to provide occupational therapy services. This may include:

• Participant name, age, address, and cultural background
• Contact details of the referrer, parent/guardian, or participant
• NDIS number, plan dates, funding type, and plan manager details
• Background information, reason for referral, and support needs
• Any other health or developmental information you choose to share

We collect personal information so that we can:

• Assess whether our services are suitable for the participant
• Provide safe, individualised occupational therapy
• Communicate with referrers, families, and funding bodies
• Meet our legal, professional, and record-keeping obligations

Personal information is stored securely using password-protected systems and, where applicable, encrypted communication tools. Access is limited to the treating occupational therapist and authorised administrative support.

We do not sell or share personal information for marketing purposes. We only disclose information to third parties when it is directly related to providing care or managing funding, such as NDIS, plan managers, schools, or other health professionals involved in the participant's care, and only with appropriate consent.

Records are kept for the period required by professional and legal standards, generally for a minimum of seven years after the last service for adults, or until a person who was a minor at the time of service reaches 25 years of age.

You have the right to:

• Request access to the personal information we hold about you
• Ask us to correct information that is inaccurate or out of date
• Request that we delete or restrict the use of your information in certain circumstances
• Make a complaint if you believe your privacy has been breached
